Necessary documents to sign up for French health insurance
from December 17, 2012 to December 18, 2014 /
To apply for and benefit from French health insurance coverage, you must visit a branch of the Caisse Primaire d'Assurance Maladie (CPAM) and present the following documents:

- Proof of identity.
- Your valid residence permit.
- Proof of residence (preferably dated within the past 3 months).
- Your first payslip, as well as your 3 most recent payslips.
- Your birth certificate, including the names of your father and mother, your birth date and location, and your gender. A birth certificate is also required for each family member applying for coverage.
Each birth certificate, if it is not multilingual or not drafted in English, German, Spanish, Italian or Portuguese, must be translated by a registered translator certified by an Appeal Court in France.

A translation made by the French Embassy or Consulate in the country where the birth certificate was issued or by the Embassy or Consulate in France of the country where the birth certificate was drawn up is accepted.
